Required Travel/Medical Insurance Forms

Haiti Healthcare Advocates, in association with the Free Methodist World Missions, requires that all travel participants obtain Travel/Medical Insurance through the VISA Ministries organization of the Free Methodist World Missions.  Appropriate forms can be downloaded and submitted using the information below:
