Travel Forms & Required Documentation

To better serve you, the following forms are available for download/submission prior to your departure. Please take the time to review all relevant material and submit the required documents as early prior to your trip as possible;

TRAVEL/MEDICAL INSURANCE

For travel within Haiti it is required that all personnel obtain adequate coverage for medical/emergency needs during the time of stay with Haiti Healthcare Advocates.  Insurance can be obtained through the World Missions organization; complete information regarding costs/mailing can be obtained HERE.

MEDICAL REQUIREMENTS AND RECOMMENDATIONS

Those professionals wishing to serve in Claire Deheureuse Hospital are required to provide a copy of their indvidual medical license and carry a copy of a form letter provided by the Medical Director.  Copies of the medical license are obtained in the Physician's home state.  A copy of the form letter can be obtained HERE.

PASSPORTS/CONTACT INFORMATION

Passports are a requirement for travel to Haiti and should be carried on your person at all times when physically traveling within the country.  Please provide your team leader with a copy of your passport, specific medical information required and a list of emergency contact information from your home country. There is a newly installed, $10.00 USD or CAD fee per person, payable at the airport prior to entering Customs.  A form for this purpose is provided HERE.
